Zieve Syndrome: Recognizing a Rare and often Overlooked Clinical Entity — A Case Report

Introduction: Zieve syndrome, a rare triad of hemolytic anemia, jaundice, and hyperlipidemia from chronic alcohol use, is often misdiagnosed as alcoholic hepatitis.
Case Presentation: A 48 years old female with significant alcohol consumption presented with fever, progressive jaundice, and vomiting. Examination showed pallor and icterus. Labs indicated anemia, thrombocytopenia,marked hyperbilirubinemia, renal impairment, and hypertriglyceridemia and positive Coombs test. Diagnosed with Zieve syndrome with coexisting alcoholic hepatitis and acute kidney injury. She improved with supportive management and alcohol-cessation counseling.
Conclusion: Zieve syndrome should be considered in alcoholic liver disease with jaundice and anemia.The cornerstone of management is alcohol abstinence.
